Connect East Ipswich covers much of, but not all, IP3 and IP4. There are approximately 71000 people in this area and was chosen because: • Social care’s ‘Supporting Lives, Connecting Communities’ programme has been successfully rolled out • Stable and experienced Adult and Community Services (ACS) team • Good fit between ACS, community health teams and GP practices • Strong GP leadership and involvement • Vibrant voluntary sector and community groups • Diverse population • Most people will go to Ipswich Hospital, which has an IP4 postcode • Manageable population size.
