Saxmundham/Aldeburgh/Leiston/Framlingham
Since May 2016, the organisations involved in the Saxmundham, Aldeburgh, Leiston and Framlingham Connect Integrated Neighbourhood Team (INT) have been working closer together to provide the right mix of support in a more efficient and timely manner to meet people’s needs.
To date, there have been many good news stories where local people are feeling the benefits and further improvements will be made as the Connect way of working continues to develop. These include:
- Working with the Local Area Co-Ordinator
Through Connect, Local Area Co-ordinator, Holly Granville supports and guides people in Leiston and Saxmundham to access services in their local area, from the Citizens Advice Bureau through to a local knitting group. She regularly links with Adult and Community Services (ACS) and Suffolk Community Health (SCH) to find out if they are aware of individuals who need her support. By services working closer together, professionals across organisations are gaining a greater knowledge of what is available in the local area, so they can help people access the services and support they need in their local area when they need it. - Working with mental health services
Making sure other public services have good links with mental health services is important to make sure that the right people are getting the right support. Through Connect, a representative from Suffolk Wellbeing Service has been hot-desking at the local Adult and Community Services (ACS) office. The benefit has been that both service practitioners are able to learn more about each other, so they have a better understanding of how the other one works and how best to access the service they provide. Similar links have also been made with Suffolk Community Health.
One of the aims of Connect is to help people be aware of what support is available in their local community to help them and their families lead a healthy and active life.
