Ipswich West (IP1 & IP2)
Since May 2016, the organisations involved in the West Ipswich Connect Integrated Neighbourhood Team (INT) have been working closer together to provide the right mix of support in a more efficient and timely manner to meet people’s needs.
To date, there have been many good news stories where local people are feeling the benefits and further improvements will be made as the Connect way of working continues to develop. These include:
- Simple Equipment Clinic – Bluebird Lodge, Ipswich
Through Connect, discussions between occupational therapists and health professionals led to a more timely solution for people who have been assessed as needing simple equipment to help them with their everyday needs.Now, there is a regular equipment clinic at Bluebird Lodge, Ipswich to help them to select, test and order the equipment they require. This saves them time waiting for an Occupational Therapist to visit them in their home.By two organisations working closer together, an additional use was found for an existing facility which now helps to speed up the process of meeting an individual’s needs.
- Drop in clinic at Hawthorn Drive Surgery
Through a conversation between Adult and Community Services (ACS) and the Practice Manager at Hawthorn Drive Surgery, an ACS drop in clinic was arranged at the surgery.Once a week, an Occupational Therapist or a Community Care Practitioner is based at the surgery so that patients can visit and speak to them about any social care needs or concerns. GPs can refer their patients to the drop in clinic or individuals can book an appointment or turn up on the day.This means that people in the area can receive support and advice from ACS face to face and in a familiar setting close to home. The ACS practitioners can offer individuals help and advice on simple equipment and adaptations and how to access care and support.
One of the aims of Connect is to help people be aware of what support is available in their local community to help them and their families lead a healthy and active life.
