Bury Rural
Since April 2016, the organisations involved in the Bury RuralConnect Integrated Neighbourhood Team (INT) have been working closer together to provide the right mix of support in a more efficient and timely manner to meet people’s needs.
To date, there have been many good news stories where local people are feeling the benefits and further improvements will be made as the Connect way of working continues to develop. These include:
- Primary Care Multi-Disciplinary Working
Through Connect, the social work team has created a Care Co-ordinator role responsible for working closely with partner organisations to co-ordinate communications between the Bury social work teams and other health and social care workers to ensure that cases are managed in an accurate and timely manner.The Care Co-ordinator acts as a consistent contact between the multi-disciplinary team meetings held in the various GP practices serving the Bury Rural area and the Bury based social work teams. This ensures that newly discussed individuals are directed to the appropriate teams and that known individuals already on the social work caseload are swiftly identified and managed accordingly.For the person involved, this means that their case is considered promptly by the correct health and social work team and necessary services delivered in a timely manner. Previous experience of delays and ‘bouncing around’ the system should be avoided. The risk of people tipping into crisis is also significantly reduced.
One of the aims of Connect is to help people be aware of what support is available in their local community to help them and their families lead a healthy and active life.
