Ipswich East (IP3 & IP4)
Since June 2015, the organisations involved in the East Ipswich Connect Integrated Neighbourhood Team (INT) have been working closer together to provide the right mix of support in a more efficient and timely manner to meet people’s needs.
To date, there have been many good news stories where local people are feeling the benefits and further improvements will be made as the Connect way of working continues to develop. These include:
- Simple Equipment Clinic – Bluebird Lodge, Ipswich
Through Connect, discussions between occupational therapists and health professionals led to a more timely solution for people who have been assessed as needing simple equipment to help them with their everyday needs. Now, there is a regular equipment clinic at Bluebird Lodge, Ipswich to help them to select, test and order the equipment they require. This saves them time waiting for an Occupational Therapist to visit them in their home.By two organisations working closer together, an additional use was found for an existing facility which now helps to speed up the process of meeting an individual’s needs.
- Joint Occupational Therapy Assistant Visits
Through a Connect networking event for the Ipswich Integrated Neighbourhood Team, an Occupational Therapy Assistant from Adult and Community Services met with an Assistant Practitioner from Suffolk Community Healthcare and discussed the similarities between their roles and how they often separately visit the same people to assess them for different needs. Now, the Occupational Therapy Assistant and Assistant Practitioner regularly go on joint visits to people’s homes to assess and provide pieces of equipment to help them with their everyday needs.This means that, instead of two visits from different teams, individuals receive a joint visit where they only tell their story once and the workers are able to cover all the individual’s requirements when needed.
